FRMS 6th and 8th grade students will be taking part in the Oregon Student Wellness Survey, sponsored by the Oregon Health Authority and the Oregon Department of Education. It will ask 6th, 8th, and 11th grade students about academic commitment, the school environment, positive youth development, and behavioral risks such as fighting,  depression, substance use, and gambling. The SWS gathers information that is used to plan and evaluate programs  that address these areas. The survey provides schools and communities a clear picture of what the problems are, what prevention efforts/programs are working, and how we can better support youth in our community.

The survey will take 45 minutes or less.  Data gathered will be anonymous. The data will be reported back in groups only- no individual data will be reported. Students will not be asked to put their name or any identifying information on the survey. Students are free to skip questions or stop at any point. Parents can also opt their child out of the survey by returning the attached letter by Thursday, February 22nd. A hard copy of the letter was sent home with students.  Students will be taking the survey as part of their Language Arts classes.
